Making A Referral

All referrals to the In-Patient Unit, Day Hospice, dementia services, Befriending, Complementary Therapy at Home and Weekend Hub must be submitted using our referral form and sent via email.

A patient with a life limiting illness:
We offer supportive and palliative care to patients who have a life limiting illness, at any point on disease trajectory. We can help with pain and symptom management, emotional and psychological support, and can discuss advanced care planning or preferred priority care.

A patient who is a carer:
We offer support and practical advice to people who are looking after a friend or relative who has a life limiting illness. We can help with 1:1 therapy, counselling, benefits advice, social work advice and spiritual support.
